Forms must be submitted prior to school attendance.
Required for all families every year:
- Health/Emergency/Waiver Form (one form per family)
- Non-Public Transportation Reimbursement (Qualifying residents may submit each year by law to receive reimbursement pay for travel expenses. The school is required to have a copy on file for participating families.)
Required for new families, kindergartners, and 7th graders (if we have this on file, we do not need another one):
- Medication Authorization (must be completed each year if any prescription or OTC medication is to be administered at school. Medication must be provided by the family.)
